Coordinator of Application Unit 

The ideal candidate will coordinate the day-to-day activities of the Agency’s Application Unit.   The Application Unit processes referrals from other service providers and individuals seeking Agency services, including walk-ins and hospital discharges. 

DUTIES AND RESPONSIBILITIES MAY INCLUDE, BUT ARE NOT LIMITED TO:

  1. Assess needs and identify emergencies of new and returning clients.
  2. Conducts emergency intakes, as appropriate.
  3. Makes appropriate referrals both within the Agency and to outside service providers.   Monitors follow-up on referrals.
  4. Coordinate and monitor the activities of the application unit, including, but not limited to verification of benefits, initial authorizations for services, fee setting, collection and  reporting of demographic and other information for statistical purposes
  5. Facilitate efficient delivery of services through coordination of the application process,
    arranging appointments and managing the waiting list. 
  6. Liaise with program staff regarding applications as appropriate.
  7. Coordinate and monitor the activities of interns/students assigned to the Application Unit.
  8. Maintain a resource file for staff and client use. 

EDUCATION AND EXPERIENCE

Qualified candidates must hold a current NYS license MSW/CSW and have experience with SPMI and MICA populations. 

Position requires excellent communication and interpersonal skills with the ability to prioritize and organize in a fast paced environment.  The ideal candidate is a clinician with excellent computer skills and the ability to multi-task.  Previous supervisory experience a plus.

ADMINISTRATIVE ASSISTANT

A full-time administrative assistant position is available immediately in the very busy administrative office of a not-for-profit mental health agency.   The administrative assistants represent the agency in every capacity, serving as liaison between clients, staff, executives, outside vendors and other concerns.   The position will handle confidential communications and requires professionalism and discretion.

  • Responsibilities may include, but are not limited to:
    secretarial support for the CEO and other senior management;
  • file organization, preparing meeting materials, etc.
  • attendance at and minutes of meetings; 
  • various administrative functions related to the smooth operation of a multi-service agency.  

The ideal candidate will be polished, communicative, and intelligent with the ability to thrive in a fast-paced environment.  Candidates will demonstrate professionalism, be willing to work independently and produce high quality work.   Specific requirements include 5 or more years experience including:

  • Strong secretarial skills, excellent typing and computer skills, including           demonstrated knowledge of all Microsoft Office applications;
  • Steno or fast longhand;
  • Ability to clearly present information through the spoken or written word;
  • Read and interpret complex information in a timely manner;
  • Take initiative, anticipate needs, adapt to situations as they arise, own the outcome;
  • Work in a team environment and provide backup as needed;

LICENSED CLINICAL SOCIAL WORKER

Licenensed clinical social worker (LCSW) for outpatient mental health program. Hours include some evenings and Saturdays. Applicant should have experience with SPMI/MICA clients. CARE COORDINATOR - ACT TEAM

The Care Coordinator is a full-time position responsible for managing medical records and operating and coordinating the management information system.  Specifically, duties may include, but may not be limited to:

  • Ensure that a complete case record is maintained for all recipients in accordance with recognized and acceptable principles of record keeping. Ensue that service plans reflect recipients' needs
  • Review clinical documents to ensure that revisions to service plans and/or other clinical documentation are done in a timely manner, using appropriate clinical language and properly reflect any changes in receipients’ needs.
  • Quality Improvement, Utilization Review, and Performance Improvement activities may include, but may not be limited to, data collection, analysis of outcome trends, and identification of service provision which needs improvement, completion of case records, and the timely submission of required documentation.
  • Plan and coordinate all services which support and supplement the rendering of case management and care coordination services to recipients. 

The ideal candidate will have a minimum of a bachelor's degree in social work, psychology, rehabilitation counseling or related field and two years work experience  in a mental health setting.  (Masters' level or higher candidates will be considered with one year experience in a mental health setting.)   Field work required.

Candidates should have a good understanding of concrete services, i.e. Medicaid, medicare, DSS and SPA applicaton. Must be computer literate, with word processing skills sufficient to maintain responsibility for their own correspondence.

SUPPORTIVE CASE MANAGER
 
The full-time Supportive Case Manager will provide and coordinate services to help clients who are disabled by psychiatric disorders to function in the community. Services are authorized by client treatment plan in mental health program.
 
DUTIES AND RESPONSIBILITIES INCLUDE:
  • Facilitate services which promote better adjustment to living in the community and entitlement to financial and other benefits.  Services are geared toward concrete needs, such as food, housing, income maintenance, child care, job placement and health care.  Prepares required documentation including case management plan and assessment reviews.
  • Assesses need for services and engages in service planning. Promotes receipt of appropriate services for each client through advocacy and service linking.  
  • Perform home visits to clients and makes trips in their behalf to other agencies. Accompanies client in arranging service or benefits when indicated.  Develops support groups for the recipient.
  • Provides emergency services to clients at home or on a walk-in basis.
  • Acts as information and referral source for agency staff and clients.
  • Prepares appropriate and prompt record keeping procedures and in preparing statistical reports to comply with Agency regulatory requirements.  
This is a full time position of 37.5 hours per week and serves a caseload of approximately 20-25 cases. 

Bachelor’s degree in a human service field - social work, psychology, nursing, rehabilitation counseling, education, occupational therapy, physical therapy, counseling, community mental health, child and family studies, sociology, speech and hearing, public administration, business administration, health care or hospital administration.

REGISTERED NURSE - PART TIME
Our PROS Program is designed to provide treatment for people who have a history of both mental illness and substance abuse. The program is seeking a part time RN for 3 days per week.  This position allows for a flexible schedule. Requirement: NYS license. Some of the duties include: administration of injectable medications as ordered; ordering and maintaining supply of Riperdal Consta; mangement of medication cabinet, using appropriate methods of disposal; and other support and follow-up services appropriate to the role of a nurse in an outpatient Mental Health Program. LICENSED PRACTICAL NURSE - FULL TIME
Work as part of a multidisciplinary team to deliver services to individuals diagnosed with mental illness. Services include, but not limited to, assistance with clinic appointments, lab appointments, obtaining random urine analysis and injections. PER DIEM RESIDENTIAL COUNSELORS
The Central Nassau Residential Program, celebrating its 20th year of dedicated service to our surrounding community, is seeking applicants to join our skilled, supportive and diverse team of friendly compassionate professionals that truly care about helping people. The duities of this position includes: 
 

  • Monitoring medications and counseling residents on medication-related issues.
  • Supervising meal preparation and clean up.
  • Offering counseling and support in recovery issues.
  • Coordinating treatment through contact with other service providers.
  • Being available for assistance in emergency situations.
  • Planning and participating in recreational activities.Facilitating positive interactions between residents.

Requirements for consideration are a Bachelors Degree, at least 1 year of experience in the human services field and a clean driving record. Prospective applicants must be available to work on a per diem/on call basis and have their own transportation.

PROGRAM ASSISTANTS
Our PROS Program is designed to provide treatment for people who have a history of both mental illness and substance abuse. The program is seeking several full time postions in both our Brentwood and Hicksville locations. The positions will be to facilitate the sign-in and sign-out of all PROS participants on a daily basis, supervise consumers during structured skill development and assist in program service activities during the program day.  This position reports to the Director and/or Program coordinator.
