Bridging the
Gap – Veterans Services

CN Guidance, through funding from the Mother Cabrini Health Foundation, bridges the mental health and substance use disorder treatment gaps for military-connected individuals on Long Island by providing connections to local psychiatrists and therapists.

This program aims to address the preventable isolation and disconnection of local Veterans and military-connected individuals on Long Island from urgently needed mental health and substance use treatment services and supports. The program recognizes that many military-connected individuals, especially those who have transitioned from their military service within recent years, are struggling with untreated depression, anxiety, suicidal ideation, post-traumatic stress disorder, and/or addiction issues.

We seek to reduce the stigma of and barriers to seeking help amongst many distressed military-connected individuals.

  • Peer Support
  • Counseling & clinical supports
  • Transportation assistance
  • Linkages to Community Resources
  • Referral & Warm Hand-off
  • Mental Health and/ or Substance-Use treatment
  • (individual and group therapy, medication management)
  • Medication-Assisted Treatment for Substance Use
  • Ongoing Case Management
  • Physical health/ Primary Care practices

Eligibility

Prior or current service in any branch of the US military for yourself or a loved one, regardless of length of service, discharge status.

Government-approved designation as a Veteran is NOT required.
