OnTrackNY

OnTrackNY is an early intervention program providing services within Nassau County for adolescents and young adults ages 16-30 who are experiencing unusual thoughts, feelings or behaviors (symptoms of psychosis). OntrackNY can meet clients in the community or at our offices.

Services

OnTrackNY offers coordinated specialty care services including:

  • Individual and group therapy
  • Assistance with education and employment
  • Peer support
  • Family support
  • Connection to community resources
  • Medication management
  • Health education/health monitoring

Is OnTrackNY right for you or your client?

  • Have recently begun experiencing changes in thinking or perceptions for more than a week but less than 2 years
  • Starting to withdraw from family or friends
  • Challenged by feelings of stress, anger, fear or confusion
