Patient Navigation Services - Project Connect

Our Project Connect team will:

  • Meet/connect with individuals and their families at Northwell Hospital Emergency Departments (EDs) in Nassau and Suffolk Counties who have experienced substance use or misuse issues that are impacting their lives.
  • Extend our existing supportive offering within in-community settings instead of waiting for individuals to arrive at emergency departments.
  • Have Peer Navigators provide support and direct connection to OASAS-licensed treatment programs,  linkages to recovery support and case management services
  • Provide, after initial contact, follow-up support at 7-day, 30-day, 60-day, 90-day, and 120 days.
